The Rising Tide: Why Malaysia Must Act Now to Protect Our Coastal Future

For generations, the rising sea was whispered about as a distant threat—a concern to be inherited by our grandchildren. That narrative, however, has expired. The water is no longer approaching; it has arrived.


From the salt-poisoned wells of coastal Kedah to the submerged streets of Taman Sri Muda, where floodwaters surged to four meters, the climate crisis has breached our threshold. As we face a reality where sea levels are projected to rise significantly by the end of the century, the safety of our homes, our hospitals, and our economic stability hangs in the balance.


A Mother of All Injustices

The Lancet Commission on Sea-Level Rise, Health and Justice has sounded a definitive alarm: sea-level rise is "the mother of all injustices." By 2100, an estimated 410 million people globally will reside on land below high-tide levels.


In Malaysia, the danger is acute and immediate. With 111 towns and cities along the Peninsular coast at risk—and Sabah and Sarawak facing even greater inundation—we are standing on the front lines. The 2021-2022 floods alone cost the nation a staggering RM6.1 billion, a stark reminder that this is not just an environmental issue; it is a profound economic and public health emergency.


The Hidden Health Crisis

The consequences of our changing coastline extend far beyond physical property damage. As temperatures climb and flood patterns shift, we see the ripple effects on human health:


Disease Proliferation: Rising temperatures are extending dengue seasons, while failing sewage systems post-flood spike cases of leptospirosis, cholera, and typhoid.


Chronic Conditions: Salinity intrusion is linked to increasing rates of renal disease and pregnancy-related hypertension in vulnerable communities.


The Unseen Toll: The mental health burden—the anxiety, depression, and trauma caused by recurring displacement—remains largely unmeasured and unsupported.


Turning Strategy into Survival

Despite the gravity of the situation, Malaysia possesses the institutional framework to lead a regional transformation. We hold one of the world’s most biodiverse mangrove systems and a maritime space double the size of our landmass.


Two critical pillars already exist to guide our path forward:


The Blue Economy Blueprint: Currently under development, this strategy aims to grow our marine economy to an estimated RM1.4 trillion by 2030. To succeed, it must prioritize the health of mangroves, seagrass, and coral reefs, recognizing them as essential public health infrastructure that defends our coasts more effectively and affordably than concrete walls.


The National Planetary Health Action Plan: Launched in November 2025, this globally recognized, science-based framework connects ecological integrity with economic resilience.


The Call to Action

Frameworks are only as strong as the commitment behind them. To secure our future, we must move with urgency:


Fund the Vision: The National Planetary Health Action Plan requires immediate political backing and dedicated funding to transition from policy to practice.


Map and Retrofit: We must systematically map coastal health facilities against sea-level projections and prioritize them for retrofitting, ensuring that the very places meant to save lives are not themselves washed away.


Plan for the Future: We need a national managed-relocation policy. Waiting for displacement to become chaotic will only deepen the costs for our most vulnerable communities.


Lead with Solidarity: Following the 2025 International Court of Justice advisory opinion, Malaysia is in a position of strength to demand loss and damage finance from the world’s largest emitters, standing in solidarity with our fellow vulnerable nations.


The former president of Kiribati, Anote Tong, once offered a chilling reminder: "We might be on the frontline today, but others will be on the front line next."


Malaysia is next. The sea will not wait, and neither can we. The time for deliberation has passed; the time for decisive, courageous action is now.
